MHS Facilities Requests
Please complete the Facility Request form below. The form will be reviewed by school administration and you will be notified if your request is granted.
Note: Before completing the request form please look at MHS Facilities Calendar (on left side) to see if the location you want to use is available for the day and time needed.
REGULATIONS FOR USE OF HIGH SCHOOL FACILITIES
- The undersigned adult members of the group agree to assume personal responsibility for the proper care and use of the facilities requested and the conduct of those attending.
- At least one of the undersigned will be present at all times.
- All groups using our building are responsible for seeing that those areas of the building used are clean when they leave. Papers and other trash are to be put in the proper receptacles.
- NO SMOKING! On school premises.
- Area of use is limited to that for which permission is granted.
- Continued and future use of these facilities is dependent on the care taken by users.
